Please complete this form to submit your application for membership in the MPA. To fulfill the application, you must send to the MPA three scores and the company’s most recent catalog. We happily accept scores and catalogs as PDFs; our administrator will follow up after you have submitted the form.
Once your application has been reviewed and approved by the MPA Board of directors you will be sent an invoice for annual dues, to be paid by check or PayPal.
